This isn't a medical form, it's a conversation starter. The answers you give here will shape the care plan we build around you, your household, and your recovery.

There are no wrong answers. Some of these questions might make you pause, and that's by design. Postpartum is a period of becoming, not just recovering. The more honestly you can tell us what you're hoping for, worried about, and working with, the better we can meet you where you actually are.

Julia has already shared the basics with us from your conversation. This form goes deeper into how you want to feel, what support actually looks like in your home, and what matters most to you in the people caring for you.

Time: About 15 minutes. We recommend doing this when you have a quiet moment, not on the go.

Q22. We can connect you with a range of wellness services that can be woven into your postpartum care. For each one, tell us where you are. You don't need to decide anything right now, "curious" is a perfectly good answer. We'll provide our curated list of vendors and resources for anything you'd like to know more about.

Massage therapy
Postpartum massage isn't spa massage. It targets the specific tension patterns of pregnancy and delivery, lower back, shoulders from feeding, abdominal recovery. Most clients say it's the single thing that made them feel like a person again.
Pelvic floor physical therapy
Your pelvic floor just did the hardest thing it will ever do. Pelvic floor PT isn't just for people with problems, it's preventive care that can change how your body feels for years. We recommend it for everyone, especially after C-section or assisted delivery.
Acupuncture
Used for milk supply support, pain management, anxiety reduction, and hormone regulation. Particularly effective in the first two weeks when your body is recalibrating.
Lactation consultant (IBCLC)
A board-certified lactation consultant who comes to your home. Different from the hospital LC who spent five minutes with you, this is someone who sits with you through a full feed, troubleshoots latch, assesses supply, and builds a plan. Most feeding struggles in the first week are solvable with the right hands-on support.
Reiki
Energy work that many postpartum clients find grounding, especially for processing the emotional intensity of birth and early motherhood. No pressure, no woo requirement. Some people love it, some don't connect with it.
Hypnotherapy
Not what you think. Postpartum hypnotherapy is used for birth trauma processing, sleep anxiety, and identity transition work. If you had a difficult birth or are carrying anxiety you can't quite name, this is worth considering.
Mental health support
A licensed therapist who specializes in perinatal mental health. This isn't crisis care, it's proactive support for a period that is, by definition, emotionally intense.
Couples therapy
The transition to parenthood (or to another child) reshapes every relationship. Couples therapy during this period isn't about fixing problems, it's about building the communication patterns you'll need when you're both sleep-deprived and overwhelmed.
Chiropractic care
Pregnancy shifts your skeletal alignment in ways that don't always resolve on their own. Postpartum chiropractic focuses on pelvic realignment, upper back strain from feeding, and general recovery support.
Section 7 — Logistics & Safety

Merges the logistics from the old Form 1 with the medical questions from the old Form 2. Kept factual and fast, you've already done the reflective work above.

Section 8 — Prenatal Baseline

Almost done. These last four questions give us a snapshot of how you're feeling right now, before baby arrives. We'll check in with you again at key points during your postpartum period. This isn't a test. It's a way for us (and you) to see how things are shifting over time.
